2. Consent Form for "Genetic Analysis of Familial Cancer in Jewish Women". This will confirm that you understand the study and your role if you choose to participate. Please read this carefully prior to your appointment. You will be asked to sign and date it at the time of your appointment. Step 2: Documents that NEED TO BE COMPLETED prior to your appointment:

You may complete and submit these surveys electronically or you may print these forms, complete them by hand, and bring them in at your appointment time. If you are unsure how to respond to a question in the research forms, please leave them blank. The study coordinator will be happy to assist you with these inquiries at the time of your appointment.
If you are completing the surveys electronically, please note that you will be allowed to save your answers if you are not able to complete the questionnaires during one session.

Special reminder: We request that you do not have anything to eat or drink within 30 minutes of your appointment time. Please remember that we are located at 790 Bay Street, 7th Floor, Toronto and not at Women's College Hospital. We strongly encourage you to come 10 minutes prior to your appointment time. Due to the high volume of patients, we will not be able to phone you with a reminder of your appointment.
